Documents will be delivered electronically to your WSU eMail whenever possible. Print copies if desired. The document will be delivered to your department's administration office within 48 hours. Deliveries are made Monday through Friday, except on holidays, at 9:00 a.m. and 1:00 p.m. If you have additional instructions and/or comments regarding the delivery of this material, please include those in the box below.

Warning Concerning Copyright Restrictions: The copyright law of the United States (Title 17, United State Code) governs the making of photocopies or other copyrighted material.
